• Your name and contact information

• Patient information (if you are inquiring on behalf of another)

• Your reason for evaluation

payment

We do not currently accept any insurance payments, all evaluations will require direct payment. An invoice for all services will be provided prior to evaluation. A sliding fee scale is available for qualifying patients. Payment is due on the date of evaluation and requests for additional documentation or testimony may result in additional fees. Appointments canceled within 24 hours will be subject to a cancellation fee.

An evaluation with our neuropsychologists does not constitute a treatment relationship, we offer consultation only. All evaluation content is confidential with some exceptions, which will be reviewed with you at the time of your initial appointment.

forensic consultation

Should you be referred for forensic evaluation (i.e. an evaluation that could be used in a court case), your evaluation will require some different paperwork to ensure it can be used in court, please be sure to let us know prior to your evaluation if you may want to use the evaluation contents in any litigation (e.g. lawsuit or disability appeal).
